New Patient Instructions

Advanced Nephrology Associates physicians and staff would like to welcome you to our practice.  We are dedicated to providing you with high quality patient centered care.

This New Patient Packet below is designed to gather information about your health history and help us better prepare for your first visit.  To save time on the day of your appointment, please complete all pages in the New Patient Packet.  The information can be faxed to 412-766-4193 or mailed to 824 California Ave,  Pittsburgh, PA 15202-2706.

We look forward to seeing you at your scheduled appointment.   If you are unable to complete the packet prior to your visit, please plan to arrive 15 minutes before your scheduled time, so we may answer any questions and you can complete the necessary forms.   If you were able to complete the paperwork but did not send it back, you may also bring it with you the day of your visit.

 

Please review our financial policy below so you are aware of any payments that might be due at the time of service.

  Items to bring to your visit:

  • Insurance Card (Primary,Secondary,Tertiary)

  • Photo ID

  • List of current medications with dosage

  • Blood Work Results from past two years

  • Recent Blood Pressure Readings

  • List of Allergies

  • Imaging Reports - i.e.Ultrasounds

  • Heart Tests - i.e. Echocardiograms

  • Urine Test Results

  • Referral information from PCP (if applicable)
